Urology Associates will gladly file charges with your insurance company. We participate with all major carriers including Medicare, Blue Cross/Blue Shield and most HMO and PPO products. It is important that you provide current and accurate insurance information at each visit.
Any out-of-pocket expenses related to your visit are due at the time services are provided.
