What to Bring to Your First Appointment

At Urology Associates we request all new patients bring the following items to ensure the best quality care is being received:

• Current List of ALL Medications
• Clinical Notes from Referring Physician or Primary Care Physician
• Photo Identification*
• Current Insurance Card
